Who We Are

Today, TMM changes lives by providing healthcare services to over 600 chronically ill, uninsured, low-income adults in South Ga. at our two clinic locations in Lyons and Graham through 3 areas of work: transformation of heart, health, and home. We have been extending a healthy hands up since 2009 by showing God’s love in action!

The Win Wall

Our Win Wall is a physical representation of the impact we have made in the lives of our patients. Each “win” represents a patient who has achieved a health-related goal or milestone, whether it be managing a chronic condition or adopting healthier habits. The Win Wall serves as a source of inspiration for our patients and staff alike, reminding us of the progress we have made and the work we still have to do.

New Patient Application

Thank you for applying to be a patient at Mercy Medical Clinic.

The following documents will be needed at your initial intake appointment:

  1. All documents for proof of income (tax returns, food stamps benefit letter, 3 months of current check stubs if you or your spouse is employed)

    If you do not have any income, you will be asked to complete a “Support Letter” at your visit
  2. A copy of your picture i.d.
  3. A copy of your social security card
